Three Easy Steps
- Search: Get in touch with your company’s Human Resources/Personnel Department for assistance in obtaining a matching gift form
- Donate: Make a cash, check, or credit card donation to Ballet Arizona
- Submit: Complete the matching gift form and submit it with your donation
Once we receive your donation and matching gift form, we’ll take care of the rest. Your company will send its matching donation directly to Ballet Arizona.
